The top search shows someone with the flag in their name, the third option is me, and I have the flag in my bio.

You can import emojis into Mastodon if you are an admin. These emojis become visible to the rest of the fediverse, and can be copied with a click of a button so that other servers can use them.

I just tested and you can search for such custom emojis as well.
