Typically it shows some status indication, has a limited ability to create connections (VPNs or hw devices + DHCP only), connect to a Wi-Fi. They also respond to requests for passwords.
Then there's the GNOME Control Center (KDE, again, likely has its own thing) that can edit connection profiles, but not necessarily start/stop them (shell does that). I think KDE Plasma has its own thing.

We maintain it just because we take the users' word for that they find it useful.

Long answer:
The previous responses seem to fall into three categories: things that are not GUI, things that are not modern and things that are neither modern nor GUI.
I believe it's because the question includes a contradiction. Modern application design focuses on what does the program do, not what is the backend. NetworkManager provides a lot of functionality that doesn't necessarily translate well to a GUI app design.
