   I'm doing some grammar weirdness (I'm not good at grammar) by eliding a bunch of unpas, because otherwise it'd be like "sexual ball and sexual stick sexual hurting", which I'm not sure is appropriate

     Reguarding the grammar, because "cock and balls" modify "torture", it needs to come first. I think "pakala unpa sike palisa" would be the normative grammar.
     
     A possibly clearer solution might be "pakala unpa pi spike en palisa" but that's non-standard and many people dislike. https://sona.pona.la/wiki/pi_X_en_Y
