   @eric Run smartctl -t short on the drive and then -a and see if you have reallocated sectors. If you do you need to replace the drive. If you don't you can readd it to your array.

     @eric Oh in that case that drive is probably dead. Is your Sata card a physical Pci card or integrated into your mother board?

     @eric I wouldn't think it's your card. But if you Toshiba drive actually does support SMART, the firmware chip on the drive might also be dead.
