   All health insurance claim denials must happen asynchronously, through a system that looks an awful lot like a grand jury (you must convince a jury of the patient's peers that the claim isn't covered)

     2. insurance claims are time sensitive since treatment may be needed ASAP to cure the disease. does your process slow things down?

     Asynchronous.

     The claim is assumed to be approved because it's assumed that the doctor isn't risking their license recommending non necessary treatment. The insurance company must prove later on that they shouldn't have to pay the bill.
