It's always a fucking scam, that's what is wrong with it. They exist to make money for the shareholders, whilst excessively inflating medical costs, not fulfil any meaningful purpose. The only way they could be beneficial to anyone (besides shareholders) is if they were non profits.

I think they should be structured like a credit union, where profit sharing is used whenever earnings are had to pad internal Savings Accounts tagged to each client. Padding for deductibles, shall we say.
I think the biggest fix for the healthcare industry happens when we deflate demand by deporting a significant population that seems to be constantly draining it.
The next fix will be doing away with the doctrine of "Treatler" or preference in treatments. "Only use the cheapest drug/treatment and don't pay for the actual cure unless the cheapest solution doesn't work first" -- Doctors should retain the sole authority on what treatments and drugs are prescribed NOT the insurance agency. The insurance agency only has one fucking job; pay for it. Not take deals or incentives from pharmaceutical companies trying to go around doctors by telling insurers to deny payments for certain treatments.
