   Its a bill for the book. Apparently Doubleday was like Columbia House.. You might not remember either so.. You sign up to a club and they send you books / music every month and then you have to pay for it or send it back if you're not interested.

   What makes this special is, the bill is a PUNCH CARD. I've obscured the address (the street no longer exists anyway). Presumably, somebody received this, fed it into a computer and was able to mark the account as paid.
