If I don't show up he just let's the next guy in. Cancellation fees are bullshit and I'm not paying.

And chances are it will remain unfilled, so it’s a loss.
Would YOU put up with someone just telling you to sit there at work unpaid for an hour? I know you wouldn’t because you value your own time, just extend that to other people who aren’t you.

The doctor just let's whoever the next person who happens to be there in next. The cancellation fee is either an inefficiency tax rolled over onto to patient, or a full on scam to bilk you for a few extra bux. I will never pay the cancellation fee.
With hotels, on the other hand, it's justified (but I still won't pay it).Token likes this. -

At the end of the day that’s still a gap in their time that’s not being monetized because you wanted it and then failed to show. It’s unwanted downtime, few pt’s are showing up an hour ahead of schedule to let the office move everything up to accommodate you. That’s just what you want to happen so you don’t think you’re causing a problem for them.
For a poorly run, corporate office, you have a point, they make you wait because they’re double booking in the expectations they’ll have enough no-shows to make it work how you say. And….that’s why doctor’s offices are so aggravating. Airlines, too.
